Filing an SSD Claim

In order to receive Social Security disability (SSD), you must take the proper steps and apply at your local Social Security office, over the phone, or online through the Social Security Administration (SSA) website. This process requires that you complete the Adult Disability Report (Form SSA-3368), which can be submitted in person or completed online, and you will also need to complete an Application for Disability Insurance Benefits at the time of application. The following documents will also have to be presented at the time of application:

  • Birth certificate or other proof of birth
  • Proof of U.S. citizenship or lawful alien status
  • U.S. military discharge papers if you served before 1968
  • W-2 form(s) and/or self-employment tax returns for the previous year
  • Medical evidence already in your possession, such as records, doctors' reports, and recent test results
  • Award letters, pay stubs, settlement agreements, or other proof of any temporary or permanent workers' compensation

What to Expect After an SSD Application

After you've completed your application, it will be sent to the Disability Determination Services (DDS) for review. They will look over your application and make a decision, either granting or denying your claim. The DDS receives millions of applications each year and has very little time to make a thorough review, which means that many valid claims are denied due to lack of proper evidence.
